We are recruiting for a Band 6 – Occupational Therapist to join our team at Glebe Care Centre. This role is 37.5 hours per week.

Confer with GP, residents and their families, nurses, therapists, social workers, and other members of a resident’s care team.
Develop a treatment plan for residents with specific goals and tasks that will help them meet those goals.
Guide residents to perform tasks to meet their goals.
Demonstrate exercises that can help residents better perform everyday tasks and relieve pain.
Evaluate a resident’s room and the Home identifying potential improvements based on their needs
Recommend and teach residents how to use adaptive equipment that will help them with daily living, such as wheelchairs and eating aids
Assess residents’ progress against goals incorporated within their careplan.
Create a programme that commits to the therapeutic value of age appropriate activities for older people
Consider appropriate activities which allow for the involvement of residents with impaired ability
Regularly promote leisure and recreation within the service.
Consult with the manager, people who use services and other staff members when planning and co-ordinating leisure and recreational pursuits.
Undertake any necessary risk assessments in connection with leisure and recreation and verify ACH’s insurance cover if required.
Ensure that the individual social needs and occupational interests of each person who uses the service are identified and logged in the Care Plan by key workers.
Take regular feedback from residents as to activities and hobbies they enjoy and incorporate them into the care plan.
Maintain accurate records of progress against goals.
Actively promote the service and ACH by clear, respectful and helpful communication with all potential customers including people who use services, relatives, health and social care professionals, other officials and contractors, visitors as well as ACH colleagues.
Contribute to the success of the whole staff team by respecting colleagues and participating in team meetings and communications systems.
Any other duties consistent with the business of ACH, the job, and hours of work as may reasonably be required.
Professional Qualification in Occupational Therapy
2 years experience post qualification
Experience of working with clients with Brain Injury
Up to date knowledge of relevant legislation
Experience working in a multidisciplinary environment
Up to date knowledge of Microsoft packages
Ability to deliver reports and meet deadlines
Affective time management and good record keeping
Excellent interpersonal and communication skills
Drivers Licence and access to a car
